Drug, alcohol treatment and non-clinical sexual health services

Drug and alcohol misuse in the Northwest region of England is the worst in the UK, and Blackpool has some of the worst levels of drug and alcohol related harm (health, disorder, violence) in the region. Months of life lost due to alcohol in Blackpool, which is the difference in life expectancy between Blackpool and England that is alcohol related, is 21 on males and 9.5 in females, the highest in England. The area has some of the highest levels of alcohol related harm in the country, not only direct health effects such as premature death and chronic liver disease, but other consequences such as disorder and violence. Furthermore, 1,700 people receiving specialist alcohol treatment each year and over 4,500 alcohol related hospital admissions annually. In relation to drug use, Blackpool has higher estimated levels of opiate and crack use of at least two and a half times the national average. Injecting drug use in the town is also estimated to be considerably higher than average at over 3 times the national rate. In view of this landscape, Blackpool Council is committed to delivering the Public Health Outcomes Framework with a view to improving and protecting the health and well-being of Blackpool's population, and improving the health of the poorest fastest. Drug and alcohol treatment impacts on two related Public Health priorities: Helping people in Blackpool to live longer and healthier lives by reducing preventable deaths and the burden of ill health associated with smoking, high blood pressure, obesity, poor diet, poor mental health, insufficient exercise and alcohol. Reducing the burden of disease and disability in life by focusing on preventing and recovering from the conditions with the greatest impact, including dementia, anxiety, depression and drug dependency. The Blackpool integrated drug and alcohol treatment system, called HORIZON, is currently commissioned to deliver these public health priorities. The aim of the treatment system is to: Improve the health and well-being of the Blackpool population by promoting and supporting recovery from drug and alcohol harms. Increase the number of clients successfully completing their treatment and supporting with their reintegration into the community. Due to Public Health transitioning from the NHS into Blackpool Council on 1.4.2013 it was a requirement that a procurement programme was put into place that included the provision of quality drug, alcohol and non-clinical sexual health services for the population of Blackpool. As the drug, alcohol and non-clinical sexual health contracts are also due to end as of 31.3.2014 these services comprise the year one programme of procurement. The aim of this tender is to commission treatment services that deliver positive outcomes, recovery, and progress within treatment and so deliver the best Public Health outcomes for Blackpool. Blackpool Borough Council is looking to redesign its existing range of Drug Alcohol Treatment and Sexual Health Services provision and would welcome submissions in response to this Prior Information Notice, which will allow the review of our existing service provision. The intention is to review our service provision to ensure we are achieving Effective Delivery and Value for Money from both our internal and external providers. By Issuing this Prior Information Notice Blackpool Borough Council are seeking a Supplier or Suppliers to assist in the design, development and delivery of a range of Drug Alcohol Treatment and Non-Clinical Sexual Health Services. We would like to invite interested parties to submit an expression of interest through The Chest (the Council's e-tendering portal) to participate in this dialogue. Your expression of interest MUST include details of your track record/ experience in this arena and provide detailed examples of contracts for similar types of work/ installation with a demonstration of how this is relevant to the Council's key objectives. Please provide this information in no more than 2 pages of A4 (4 sides) no later than Monday, 10.2.2014 17.00 pm. This information will form the basis of how the Council will choose the participants to engage in further dialogue. Please note: clarification questions are not appropriate at this stage, Blackpool Council require only a written response from your organisation with the details requested above. Please note participation or (or non participation) in dialogue will confer no advantage or disadvantage should the Council commence a formal procurement process. The following Details provide an indication of the specifics of each Lot/Category of service Lot 1 Adult Drug and Alcohol Recovery Service Aims Helping people to live longer and more healthy lives by reducing preventable deaths and the burden of ill health associated with smoking, high blood pressure, obesity, poor diet, poor mental health, insufficient exercise, poor sexual health and alcohol. Reducing the burden of disease and disability in life by focusing on preventing and recovering from the conditions with the greatest impact, including dementia, anxiety, depression and drug dependency. To reduce the destructive impact of drug and alcohol misuse to those misusing drugs and alcohol, their families and the communities within which they live. Objectives. Primary Objectives: — Increase the number of individuals who are drug and alcohol free; — Reduce the intergenerational harms of alcohol and drug use; — Services to have a Family approach; — Individuals are able to maintain family and social relationships; — Reduction in drug and alcohol related crime; — Timely access to appropriate personalised treatment pathways; — Reduce alcohol related hospital admissions. Secondary Objectives: — Deliver Community Based Treatment at a range of locations suitable and convenient to the client. — Increase the number of people in or gaining employment. — Increase the number of people engaging in training and education. — Prevent and reduce (ameliorate) the harm caused by substance misuse at an individual, family and economic level. — Individuals well informed regarding the drug/alcohol problem and interventions to support recovery. — Prevent Young Adults shifting to more serious drug and alcohol taking (harm minimisation/harm reduction). — To reduce the number of children in care by the use of Long Acting Reversible Contraception in female clients. — To improve the overall health and well-being of clients including reduction in alcohol consumption and smoking cessation. Expected Outcomes Including Improving Prevention. The overall aims of the specialist community treatment programmes will be: — To promote recovery from drug or alcohol use. — Where appropriate to stabilise the clients, on substitute medication to alleviate withdrawal and to reduce craving. — To provide rapid access to specialist treatment within the waiting time targets. — To reduce the use of illicit or non-prescribed drugs. — To reduce problems related to drug and alcohol misuse, including health, social, psychological and legal problems. — To improve overall personal, social and family functioning. — To reduce the dangers associated with drug and alcohol misuse, including the risks of HIV, Hepatitis A, B and C and other blood-borne infections and the risks of drug-related death. — To offer an accelerated vaccination programme of Hep B with information about the disease and advice on how to prevent transmission –(added in by HH). — To reduce the duration of episodes of drug and alcohol misuse. — To reduce all crime associated with drug and alcohol use. — To reduce re-offending. — To reduce the risk of prescribed drugs being diverted onto the illegal drug market. — To assess the needs and safety of children living with problem drug and alcohol users and to provide access to appropriate support. — To assist the service user to remain healthy until, with appropriate support, he or she can achieve a drug-free/safer drinking or abstinent life. — To reduce the number of unwanted pregnancies and children taken into care. — To work with mental health service clinicians and clinicians supporting people living with a learning disability to deliver a collaborative service for patients with a dual diagnosis, to promote recovery. — To develop a non-medical community detox programme to support clients who do not wish to be prescribed. — Work with all partners to ensure sex workers are supported and access appropriate drug & alcohol and sexual health services. Lot 2 Clinical Service Specification. Aims Helping people to live longer and more healthy lives by reducing preventable deaths and the burden of ill health associated with smoking, high blood pressure, obesity, poor diet, poor mental health, insufficient exercise, poor sexual health and alcohol. Reducing the burden of disease and disability in life by focusing on preventing and recovering from the conditions with the greatest impact, including dementia, anxiety, depression and drug dependency. The overall aims of the Clinical Intervention Service will be: — To promote recovery from drug or alcohol use; — To stabilise the service users, where appropriate, on substitute medication to alleviate withdrawal and to reduce craving; — To improve overall personal, social and family functioning; — To provide community based drug and alcohol detoxification services; — To provide rapid access to specialist treatment within the waiting time targets; — To reduce the use of illicit or non-prescribed drugs including Benzodiazepines and over the counter medicines; — To reduce problems related to drug and alcohol misuse, including health, social, psychological and legal problems; — To reduce the dangers associated with drug and alcohol misuse, including the risks of HIV, Hepatitis A, B and C and other blood-borne infections and the risks of drug-related death; — To offer an accelerated vaccination programme of Hep B with information about the disease and advice on how to prevent transmission –(added in by HH); — To reduce the duration of episodes of drug and alcohol misuse; — To reduce all crime associated with drug and alcohol use; — To reduce re-offending; — To reduce the risk of prescribed drugs being diverted onto the illegal drug market; — To assess the needs and safety of children living with problem drug and alcohol users and to provide access to appropriate support — To assist the service user to remain healthy until, with appropriate support, he or she can achieve a drug-free/safer drinking or abstinent life — To reduce the number of unwanted pregnancies and children taken into care; — To work with mental health service clinicians and clinicians supporting people living with a learning disability to deliver a collaborative service for patients with a dual diagnosis, to promote recovery; — To deliver cost effective prescribing; — To support the case co-ordinator to deliver a non-medical detoxification programme; — Work with all partners to ensure sex workers are supported and access appropriate drug & alcohol and sexual health services. Lot 3 Drugs & Alcohol Adult Harm Reduction, Assertive Outreach together with Non Clinical Sexual Adult Health Harm Reduction Services Aims: To develop an integrated and sustainable approach to harm reduction across Horizon to improve the lives of the population. The use of peers/buddies to support this process is highly encouraged. Helping people to live longer and sustain healthier lives by reducing preventable deaths and the burden of ill health associated with smoking, high blood pressure, obesity, poor diet, poor mental health, insufficient exercise, poor sexual health and substance misuse. Reducing the burden of disease and disability in life by focusing on preventing and recovering from the conditions with the greatest impact, including dementia, anxiety, depression and drug dependency (Substance Misuse). Ensure that harm reduction is embedded within Horizon by working in partnership to ensure all clients and treatment naïve have prompt and flexible access to needle exchange services, vaccinations, testing and access to blood borne virus (BBV) treatment and treatment for Hepatitis C. Ensure the workforce is competent to deliver effective and sustainable harm reduction practices and interventions in a proactive manner. Encouraging staff members and peers to take up the mantle of harm reduction champions is highly encouraged i.e. Naloxone Champion, Needle Exchange Champion etc. Ensure that action is taken to reduce the number of drug related deaths by way of exploring and embedding innovative harm reduction interventions such as ‘Take Home Naloxone' within a client's treatment journey as a standard package of care. To contribute to the reduction of undiagnosed HIV/BBVs and sexually transmitted infections (STIs).To support survivors of sexual violence and those living with and affected by HIV. To provide harm minimisation and education to identified “at risk” groups. To reduce risk taking behaviour and promote healthy behaviours through psychological therapies, including Cognitive Behavioural Therapy (CBT). Objectives: Harm Reduction and Alcohol/Drug Assertive Outreach Objectives Primary Objectives: — Effective and sustainable reduction in the harms caused by drug and alcohol misuse to Horizon clients, treatment naive and the wider population. — Assertively outreach across Horizon to engage with those who drop out of treatment and instil motivation amongst those who find treatment difficult. — Reduce the number of drug related deaths. — Deliver interventions using a multi skilled workforce who can work across all elements of the service. — Reduce alcohol related deaths and hospital admissions. — Work in partnership with the all partners within Blackpool Victoria Hospital. — Robust screening and vaccination programme against BBVs for all those at risk following NICE guidance. — Ensure clients who test positive for Hepatitis C are referred to treatment in a supportive and timely manner ensuring support is offered to each client alongside the Viral Hepatitis Nurse for the entirety of their treatment. — To offer an accelerated vaccination programme of Hep B with information about the disease and advice on how to prevent transmission. — Reduction in drug and alcohol related crime. — Timely access to appropriate treatment. — Harm reduction initiatives must be on an ‘opt out' basis with valid reason for declining an intervention. Secondary Objectives: — Individuals are able to maintain family and social relationships. — Deliver community based treatments including home visits where appropriate. — Services to have a family approach and work collaboratively with children's services. — Prevent and reduce the harm caused by substance misuse at an individual, family and economic level. — Ensure relevant and up to date information is shared with all partners to minimise the harms caused by risky behaviours. — Prevent a shift to more serious drug taking. — Challenge the stigma that can be associated with dependence and that can act as a barrier to reducing harms. — Explore new ways of preventing the harms caused by at risk behaviours based on local, national and international evidence. — Ensure smooth entry into specialist treatment services from hospital. — Respond to address emerging harms associated with new drug misusing trends and patterns. Sexual Health Objectives: — To promote CBT and other therapeutic models to promote sexual health behaviour change — To increase levels of screening of HIV/BBV and Chlamydia — To support those screened for HIV/BBV to access service available for both a negative and positive result and reduce the stigma associated with these. — To raise awareness of sexual health. — To prevent sexually transmitted infections (STIs) in populations at high risk of poor sexual health. — To work with partners to provide support to vulnerable groups. — To increase support and opportunities for the LGBT community. — To provide a responsive, professional and independent counselling and advocacy service for people experiencing and survivors of sexual violence. — To reduce social isolation and discrimination experienced by target groups. — To actively support The National Chlamydia Screening Programme to reduce Chlamydia in the under 25 year olds. — To actively engage with and increase uptake of long acting reversible contraceptive (LARC) in women attending the service. Expected Outcomes Including Improving Prevention Harm Reduction and Alcohol/Drug Assertive Outreach Outcomes — To reduce drug and alcohol related morbidity and premature mortality across Blackpool. — Actively promote safer injecting practices to reduce drug related deaths and prevent the need for clinical interventions as a direct result of bad injecting practices. — Ensure that all high risk injecting drug users in the community and those released from prison receive take home naloxone (THN) as part of their treatment package. An individual must ‘opt-out' with valid reason. — Develop and support peers to engage, train and sustain positive harm reduction initiatives in those at risk throughout their treatment journey. — To reduce drug related deaths and other physical health condition related to substance misuse. — To reduce the dangers associated with drug and alcohol misuse, including the risks of HIV, Hepatitis B and C and other blood-borne infections and the risks of drug-related death. — Develop Viral hepatitis lead within the service to support and actively link with the viral hepatitis lead at Blackpool Victoria Hospital (BVH). — To work with the BVH viral hepatitis lead to coordinate and support clinics in a variety of settings. — Increase the uptake of screenings and vaccinations for HIV, Hepatitis B and C across the treatment system utilising dry blood spot testing. — To reduce readmissions to hospital for alcohol related causes by providing adequate specialist support in the community. — Ensure that an effective database of all clients within and entering treatment is developed across the Horizon ensuring all IV drugs users are screened for Hep C according to NICE guidance. (Utilise Poppie system). — To embed the uptake of long acting reversible contraception (LARC) for all eligible female patients and promote positive sexual health with all clients. — To provide effective harm reduction advice, materials and deliver information and support to reduce the harms to all drug and alcohol misusers. — To deliver and coordinate the needle exchange services across local ph
