The scope of this tender is: 1) through an innovation partnership, design, develop, test, commission, deliver, operate and maintain a service-based system to strengthen the prevention in chronic disease management, through remote patient monitoring and call centre services, for a period of seven (7) years, of which a maximum of two (2) years will be dedicated to the research and development Stage (see Part C of the Memorandum of Information (Annex I); 2) following purchase of the final solution, deliver the service to the patient-base of the health system, wherever they are on the Maltese Islands; 3) deliver an integrated logistical support package, including all necessary software, hardware and human resources; 4) deliver, a continuous training and support programme for the health care workers, parents and patients.
