NHS Derbyshire County has sought interest from suitably qualified and experienced providers to provide GP services in Creswell and Langwith. We require innovative, forward thinking providers to explore the service delivery model and specification of required services and to support achievement of our vision. To give you an idea of services we will require below is an outline list of services the Provider shall be expected to provide all but not be limited to, the following elements of service for which the following are a minimum requirement. (a) Essential Services, additional Services and Enhanced Services during Core Hours for all patients within the Practice Area. (b) Home visits for all Patients during Core and Extended Hours as the provider deems clinically appropriate. This shall include patient residing in care homes within the Practice Area. (c) Pre-booked access to a health care professional within 24 hours. (d) Pre-booked access to a GP within 48 hours. (e) Appointments that can be booked at least 4 weeks in advance. (f) Sufficient capacity as regards appointments and skill-mix to enable patients to make same day or pre-bookable appointments. (g) A consultation time with a GP for a routine appointment of not less than 10 minutes. (h) An effective triage system to ensure Patients receive the most appropriate clinical intervention. (i) A multi – disciplinary service led and supported by GPs as part of the integrated service delivery. (j) Access for patients to co–located services within the health community. (k) The provider is required to participate in GP and practice manager forums and other health and medical meetings to ensure appropriate integration of service delivery and whole system thinking.
