Since 1.4.2013, the responsibility for improving Public Health has transferred from the NHS to Local Authorities. Luton Borough Council is therefore inviting applications from suitably qualified and experienced organisations to provide an Integrated Wellness Service bringing together existing lifestyle services under one provider to improve the physical, mental, social and emotional health of Luton's residents and reduce health inequalities. Key service objectives: — provide advice, guidance and interventions to address key lifestyle issues, — provide a single point of access with multiple delivery points, — provide holistic assessment of individuals to identify needs, — provide support to change behaviour using structured case management approach, — address health inequalities through working with communities where needs are greatest and health is poorest, — develop links with other services and organisations to address wider health issues and social issues impacting on health, — empower service users to build resilience and increase independence.
