Provision of a Dorset Health Improvement Hub.

Scope and aim: On 1.4.2013 the responsibility for public health services within the County of Dorset transferred from the NHS to the local authorities of Dorset County Council, Bournemouth Borough Council and the Borough of Poole, all working together as Public Health Dorset with Dorset County Council being the host authority. Dorset County Council, on behalf of Public Health Dorset, is seeking tenders for the creation of a Dorset Health Improvement Hub that will provide a single point of access, a front door, for all health improvement services and referrals across the County of Dorset. This presents an opportunity to transform health improvement services in line with the legal requirement to improve health and wellbeing and reduce inequalities in health for residents in the County. The Hub shall support the engagement, assessment, referral management and follow up support available to people wanting to change health behaviours. Above all, to develop a new approach that recognises and assesses people's different attitudes to health, and whether they require support with capability, opportunity or motivation to change, each of which can be a driver of behaviour. The Council is offering a contract that will be a mixture of core funding plus a payment by results element based on activity and outcomes within health improvement pathways and up to a period of 7 years. The specification has been developed in consultation with a range of parties but the hub will focus on the core elements of customer service, client engagement and motivation, referral and management of health improvement pathways with generic low level behaviour change support. This will include provision of brief interventions for healthy choices, smoking cessation, physical activity and alcohol but not more specialist or intensive support. Seeking a provider that has the ability to deliver excellent Customer Relationship Management (CRM) and secure Contact Management Software (CMS) that will help support the behaviour change of individuals across a range of health improvement services and facilitate reporting of outcomes over time. The Council welcomes consortium bids, though the contract will be with a lead provider. Contract period: The Contract to be for an initial period of 3 years, with provision to extend up to 4 years within break points (subject to satisfactory performance); making a total of 7 years. This will potentially allow for a return of investment for both Public Health Dorset and the successful provider.
