Support to Live at Home Round 4.

The Support to Live at Home service is for: twenty four hour home care provision for older and disabled people (including those funded by Health for ongoing long term health conditions); end of life care at home; dementia care; and reablement. The work does not include: provision for people with a learning disability; provision for people with poor mental health; or provision for people funded by health and social care due to complex needs. There are approximately 2900 home care hours delivered in this area currently per week.
