The Authority is seeking to commission the Services in support of Discharge to Assess Pathway 3 on behalf of NHS South Warwickshire Clinical Commissioning Group (SWCCG). Discharge to Assess (D2A) is designed to reduce unnecessarily prolonged acute hospital stay. It is a multi-agency approach between the Authority and SWCCG including Continuing Health Care, which allows assessment for ongoing services to take place after discharge from hospital. For this contract Warwickshire County Council will act as the lead commissioner for NHS South Warwickshire Clinical Commissioning Group. SWCCG is seeking up to 30 places in total (subject to actual budget), shared evenly (i.e up to 10 places) in each of the following 3 areas: — Area 1 — Warwick and Leamington Spa: Postcodes — CV31, CV32 and CV34, — Area 2 — Kenilworth: Postcodes — CV8, — Area 3 — Stratford-upon-Avon: Postcodes — CV35, CV37, CV33 and CV47. Note: Only applicants who propose homes that are in the Postcodes listed above will have their submissions evaluated. Applicants whose homes are outside these areas should not apply as they will not be eligible for an award. The beds are to be contracted on a block basis and the number of beds in the block arrangement will be reviewed at least annually and as a result may be adjusted, either up or down, to match demand and available funding. To ensure the efficient working of the Multi-Disciplinary Team, the GP and the Hospital, the preference is that we award a minimum of 5 places per home. Lesser numbers of places will be considered if we are unable to achieve this number: subject to actual budget or lack of availability. During the contract the services may be reconfigured or new services introduced to improve the effectiveness of this service, which may require a variation. The required changes to the service delivery model will be by agreement between the Authority and the Provider.
